Patient Access
The Practice is pleased to provide patients with access to its Clinical System via a Secure Web Interface.
To use Patient Access you will need a Practice ID number and Access ID number which is obtained from the practice, and, in conjunction with a password, will be unique to you.

Please visit Patient Access where you can book appointments, request prescriptions and view your medical record online.
Please fill out our Online Access Form (DOCX, 19KB) (for age 16 and over) and send this back to us on our email with a copy of your Photo ID - nelondonicb.newburygrouppractice@nhs.net
Please note you will need to attend the surgery with your photo ID to collect your password once your form has been submitted.
Proxy Access
PROXY online access form to be used for:
- A patient who is aged under 11
- An adult who lacks capacity (we will use the medical records to determine if proxy access appropriate)
